Hudson Valley Regional Community Health Centers (HVR Community Health Centers) is comprised of a network of comprehensive health care providers in a high-quality, cost-effective, and evidence-based manner available to all members of our community

As a patient, you are the most important person in the health care system, with access to total care with a team that includes Physicians, Physician Assistants, Nurses, Nurse Practitioners, Social Workers, and Behavioral Health Clinicians.

These trained professionals coordinate your care by obtaining test results, referring to and communicating with specialists, assisting with accessing community resources, negotiating insurance plans, and overcoming other obstacles to care.

The mission of Hudson Valley Regional Community Health Centers, Inc. is to provide high quality, primary medical, specialty medical, rehabilitation, dental and ancillary health services to patients, to include those who are underserved with a special focus on the needs of people with disabilities and chronic health conditions.
